This is a survey for reporting safety incidents and injuries sustained while participating in SAKE activities. It is NOT for reporting safety concerns.
Example of a safety incident: Capsize, falling out of the boat, tripping on the dock, etc.
Example of a safety concern: Loose nail on the dock could cause an injury and should be repaired. Use the equipment survey to report things of this nature.
In essence, the difference between the two is that this report is for an actual happening vs something that could happen.
In instances of an incident involving team boats (dragon boat, OC6), this report should be filed by the steersperson, coach, or other safety officer in charge of the session.
